Annual dues are $250.00 for both new and renewal memberships. The membership and renewal membership application is available below, just click JOIN NOW. It can be printed and mailed or completed electronically by following the link below. They can also be printed and mailed to the Treasurer if needed.
We welcome any member to invite as a guest, any colleague, nurse practitioner, or physician assistant from your office to dinner lectures. The fee per guest will be $75, and a RSVP is required.
If you are a renewing member, please use the online form to update your contact information (especially email) annually.
For those of you who have not yet paid dues, Membership requires the completion of an application annually. A $250 membership fee is required for one year or four dinner/lectures. If you wish to bring a guest ( P.A. or nurse practitioner) to a dinner, a $75 fee along with an RSVP is required. If you are a renewing member, you need only to fill out the first four lines of the application and return to the current treasurer.
